I suspect it started as the second and turned into the first.

There is/was one on display at Ain Ghazal, and the premium house purge involved two Gor'Tog brothers using them.

I have a couple of ideas for how to make it work and approval to attempt to do so, it's just not something that's floated up to the top of the priorities list yet. Part of that is because instrument code is scary and part of it is because I struggle to see it as anything more than a novelty.
