450 is the minimum amount of Empathy to see transferred wound reduction on external fresh wounds. You need more than that to start reducing internals, external scars, and internal scars. At that level of Empathy, the wound reduction would be very, very minimal. It might not even show up at that level due to rounding errors, etc.

I'm not going to put all the numbers out there (unless I already did, if so, oops...) but it takes 1050 Empathy to cap the reduction of all types of injuries (which is, yes, still 1/2.)

It's also harder to reduce wounds that are being transferred quickly, and easier to reduce wounds that are being transferred slowly. So transferring slowly will allow you to reduce external wounds slightly earlier than 450 ranks of effective empathy. And then of course you'd need more than 1050 empathy to cap wound reductions for quickly transferred wounds.
