Excellent question, I'm going to say your best bet over all is to get it registered on the character that's holding it. I'd bet they could still replace it, but might take a bit of extra time to look up and reference everything, and that doesn't sound very fun for anyone involved.

But what this question makes me think of is that I have an altered item, purchased from another player, only just now recently I've realized they registered it. So I cannot. Since then I believe they have quit playing the game. Removing any chance for me to find them and have them un-register it. Wondering where that falls in the grand scheme of things.

If you are planning to keep the item on the alt character then yes, you should register it to THAT character.

If you've purchased an item already registered, and the person who has registered it is gone, then .. well. Buyer beware? Check first! As an aside, it might help to post somewhere asking for the person you bought it from. It's entirely possible they just play a different character now, or play opposite your own hours.
